Considering that our patient J.P is facing enough woes at home and at school, I feel it would be best to become acquainted with J.P first before starting a physical assessment. Looking at the physical self, in my professional opinion is not a determination of overall health. I would talk to him and see if he had any complaints first. I would allow him to open up about his Sickle Cell Disease and allow him to express his pains and pain management. With this information, I would tell him that my assessment is to satisfy the requirements for employment.
According to Jarvis (Jarvis, 2016) the order that the physical inspection must be completed in is: Inspection, Palpation, Percussion, and Auscultation. Understanding that Sickle Cell (2016, September 12) is a not a latent disease is the first step in assessing this patient. Armed with the knowledge that this patient has suffered with this malady since his birth, there should be distinguishable chronic conditions.
Documentation in the EHR will be a direct reflection of my assessment and my clinical findings. Inspection of the integument, nails, hair, eyes, and body posture will expose pallor, discoloration, alopecia, or joint damage from the disease process itself. Professionally, I would sit down with my client and ask him, “What struggles does he face daily about his place in this world,” and listen. This open line of communication (Jarvis, 2016) would hopefully breach the wall protecting his feelings.
