Daily, nurses are exposed to different situations in which decisions are to be made. “Philosophy helps nurses to think more critically and reflect on how their own values influences their practice and way of being” (Bruce, Rietze, & Lim, 2014, p. 65). Understanding the philosophy behind one’s values and beliefs will help guide the nurse to make decisions that are consistent with their beliefs and values. Early on, nursing education focused on skills and lacked the knowledge behind the why. The education took place in the hospitals with the MD’s. It slowly transitions to the universities with a focus on learning vs. training. Graduate studies started to emerge. Nursing was on the rise to be recognized as their own discipline. Since science has specific criteria to evaluate credibility, this does not always factor in the human component that is often seen in nursing practice. Grand theory is a generalized theory in which can be applied to a broad range of nursing. It is abstract and created through observations or insights of the theorist. The grand theory is utilized to address the nature, mission, and goals of nursing care.
The mid-range theory gets more specific with the patient types, less abstract, and more focused on concepts. Nurses that are drawn to theory-based research and practice tend to gravitate to the middle range theories. Many of these theories will be applied to a diagnosis, an emotion, or belief. Many clinical practice guidelines have been derived from middle range theories. This is most commonly used.
The micro-range also referred to as the practice theory gets even more narrow in its focus and applies to a very specific population. A mid-range may look at “Adult Self Care” where micro-range theory may be “Adult Self Care of Individuals with IDDM”. These theories are considered goal oriented and research based.
