On 2/22/17 I visited Mr. Rasak at The Regency. This visit was approved by the adjuster Ms. Castle. Mr. Rasak, was sitting in a wheelchair. He was dressed. He reports he is doing good. He uses the wheelchair to go down to the dining room, exercise in the halls and independently take himself to the gym for therapies. He denies pain. He is wearing his cam boot. Mr. Rasak became tearful when speaking about going home. He is aware that he must be able to transfer himself and use the bathroom independently. I spoke with the physical therapist. She reports they again tried a scooter and a walker with a knee support since he is still non-weight bearing. Mr. Rasak is unable to safely maneuver these. He is still stand by assist when transferring. …show more content…

He is able to walk using a 50% weight bearing to the right foot and a walker. He was able to maneuver the two small steps to the front door. Once inside the home he was able to sit in his lazy boy chair. The bathrooms in the home and bedroom were evaluated. The following is a list of DME Mr. Rasak will need:

He will need home physical and occupational therapy, 4 hours per day of attendant care. There may be a need for transportation.
I have contacted Incingo for DME; all will be shipped at the latest by Monday. Installation of the grab bars may occur after that. Attendant care will be provided by Acclaimed home care. I have contacted MSW at The Regency. We are targeting a release home on 3/14/17.

Mr. Rasak was discharged home on 3/14/17. Acclaimed homecare has begun to provide attendant care services 4 hours per day. I have spoken with Mrs. Rasak several times. She is very overwhelmed by Mr. Rasak. He wants her to provide all care. She continues to work outside of the home. I have encouraged her to use the attendant care services to make things

Having reviewed Mr. Finley’s medical information, as his primary care physician over the past five years, I can verify, without providing a definitive diagnosis, that Mr. Finley suffers from an ongoing and permanent, lifelong disability which substantially limits the following major life activities as defined by the ADA Amendments Act of 2008: Performing manual tasks, concentrating, sleeping, thinking and working. Additionally, the prescribed medical treatment he receives to mitigate the aforementioned results in side effects which substantially limit the very same major life activities for a period of 7 to 8 hours following treatment. The side effects caused by medication and/or treatment that a patient must take because of a disability are limitations resulting from the disability. Reasonable accommodation extends to all limitations resulting from a disability.…
