(S) “I feel empty”
LL reports an increase in feelings of emptiness triggered by witnessing decline in his own medical health. He showed writer a list of the medications that he currently takes and stated feeling like a “dumpster of pharmacy and therefore a human failure” for having reach a stage in his development where he has to be dependent of these drugs to survive. He expressed the benefit from having sessions with writer as a way to diminish lonely feelings. He continues to report an interest in working on increasing social interaction in order to feel less isolated.
(O) Appropriately groomed, appears stated age, calm, polite, cooperative, sad affect, intermittent eye contact throughout the session. He appears more depressed than a month ago but denies SI intent or plan. The church has been giving him more responsibilities than before but he …show more content…

Clinical Impressions: At the moment, client seems to be having a hard time accepting his health status and finds it difficult to see anything positive in his current life. He exhibits helping-rejecting behavior and does not accept reframing too easily. He may be more in tune with the limitation of his medical condition, and in turn is experiencing the grief of the loss of independence and the loss of the possibility of ever reuniting with an object of love.
(P) Rescheduled for 6/21/17 @ 3:30pm (client cannot meet with me this week because he has a Broadway show and dinner with friends); Pt reminded of Sage’s intention of continuing mental health services after our work in done. Continue to validate client’s feelings while offering a safe space where client can come to terms with his condition, decrease anxiety and fear of dying and improve his depressive state. Provide supportive treatment towards the solidification of ego strength so that the client experiences an improvement in depressive
