D-Client arrived on time and alert. The client engages throughout his session and participated in completing his initial tx plan. The client reported that he had prior detox treatment at Providence House twice. During his second treatment at the detox center, he lost his employment. According to the client, losing his employment had put a strain on him financially to the point that he lost everything such as his house. The client currently resides with his mother. The client reported to having a positive relationship with his mother, not his father. There is no communication between the client and his father, even though, according to the client, his father lives 10 minutes away. Furthermore, the client mentioned that his s/o is also an addict and is currently engaging in treatment at Providence House.

The client reported that he has an 11 year old son, who is in the 5th grade.

The client denied any mental health and medical issues. When asked by this writer as to how he was introduced to using illicit drugs, the client says, “I started 2-3 years. I was first introduced when I started abusing Percocet due to issues with my back. I’ve had back pain for a long time. When the Percocet stopped being prescribed, I then started using heroin. I never thought I would be doing this, but it was helping me cope with my pain.” The client did not expressed if whether or not he is still having back pain.

A-Based on this writer’s assessment, the client struggle with his craving and tend to use drugs as a substitute. The client recognized substituting drugs to help with his craving is not healthy. The client is aware that he needs to find other alternatives for substitution and is also aware that he needs to resist cravings. There was no evidence of SI/HI. All component MSE were WNL
