The client is 62 y/o who is awake and alert and who appears healthy and looks his stated age. The client’s vital signs are normal; a Body temperature of 37.5, Blood pressure is 110/80, Pulse rate is 80 beats/minute and Respiration rate of 14 breaths/minute. The circumference of the client’s head is 16.5 cm, his hair has a normal texture and have no presence of lesion and dandruff. The client can figure out the letters in the sneller’s chart and can follow the light of the penlight which indicates that the client has a good vision. The client’s pupil reacted as well in the light and has a brown iris. There was no inflammation present in the eyes. The client successfully distinguish sounds we made, and there were no lesions and masses at the client’s ears.

The client have dentures and his tongue has no lesions also and his gums, his lips are pinkish and moist without lesions, the clients throat were normal there were no inflamantation present. There were no edema,masses and scars on the client’s neck, it is symmetrical and be can bend it without feeling in pain and the client can swallow easily. The client’s skin have scars because of the mosquito bites and he has a strong muscular strength.
