Patient With Suicidal Ideation

The patient is a 14 year old female who presented to the ED with suicidal ideation to harm herself. Patient wrote a note informing her mother that she would kill herself. Patient denies HI or symptoms of psychosis.

During the time of the assessment the patient was awake and alert. The patient's mother soon after came into the room after the assessment started. The patient reported that she had been recently skipping school with her boyfriend. Patient expresses feelings of worthlessness. Per documentation the patient reports that she feels like no one wants her. The patient's mother reports that she found out that the patient has skipped school twice this past week. The mother expresses that her daughter has had issues at school with rumors and bullying a year ago and is receiving services at NC-Mentor in Asheboro, NC. The patient's mother reports that the patient wrote a not to her about killing herself if she kicked out the boyfriend who lives in the home with them. The patient reports that recently she has been having suicidal thoughts. Patient reports 2x attempts in her past. The patient's mother reports the patient has practice cutting behaviors in the past/ The patient reports that she has cut in attempt to hope she would bleed out from the multiple lacerations on her up legs and arms in 2014. There are no visible cut mark or superficial cut on patient. The patient reports Marijuana use, which was confirmed by her most recent drug screening.

TACT consulted with Dr. Gentry and it was recommended to refer for inpatient hospitalization for safety and stabilization. TACT assisted the ED doctor in completing IVC paperwork. TACT will search for appropriate
