Patient reports restlessness or feeling keyed up or on edge, difficulty concentrating, and sleep disturbances (difficulty falling or staying asleep). Patient reports at times she feels chest palpitations secondary to her anxiety. History of substance abuse, current tobacco abuser, denies use of alcohol or illicit drugs at this time.
Patient reports depressive moods, denies thoughts of suicide or homicide. Patient denies chest pain, SOB, N/V/ D, or fever.
