D-The patient arrived on time for his appointment and this writer agreed to allow the patient to dose first before his session. The patient received a dose increase this past Saturday and reports stability with his current dose thus far. According to the patient he admits to not using for two weeks, but relapse on Father's Day. The patient used heroin (2 bags by IV) and cocaine (1 bag by IV) .When asked what caused his relapse, he says, " Someone given it to me." This noticed the patient struggles with people, place, and things as a trigger and discussed alternatives. This writer then addressed alternative and then begun informing the patient that his ongoing use needs interventions whereas alternatives has been addressed, but not working.
