The Institute of Medicine has released a report to transform U.S. health system by emphasizing the six aims of improvement for the hospital setting. These six aims are safety, teamwork and collaboration, patient centered care, evidence based practice, quality improvement, and informatics (Cherry and Jacob, 2014). Safety can be implemented through minimizing risks factors. For example, patient with fall risk can be on eye sight and provide to wear a special yellow sock that grips better. Teamwork and collaboration focuses coordination among health care professional to improve patient care. These are identifying patients correctly, improve staff communication, use medicine safely, use alarm safely, prevent infection, identify patient safety risks, and prevent mistakes in surgery. To identify patient correctly the National Patient Safety Goals recommends that nurses should use at least two ways to identify patients (Cherry and Jacob, 2014). For example, use the patient’s name and date of birth. This way right patient will get right medicine. To improve staff communication nurses should collaborate with other health care providers in their work environment in a timely manner (Gardner, 2012). For instance, if patient has a blood work, the test result should be done in right time and send to the right person with the right channel. Medicines should be used safely. This can be obtained through labeling each medicine, educating the patient to make sure they know which medicine they are taking. Alarms should be used safely by improving the efficiency. To prevent infection guidelines from the Centers for Disease Control and Prevention or the World Health Organization should be followed (Cherry and Jacob, 2014). All staff members and nurses should follow hand wash procedures. This procedure should be explained to them and observed without their knowledge to prevent hospital based illnesses and improve the patient care. Identifying patient safety risks through risk assessments to prevent suicide should be mandatory. When nurses implement their knowledge of evidence based practice and utilized their experiences, they may help to reduce the risks for the high-risk suicide patients.
