Healthcare costs in the United States are higher than those of any other country yet the evidence does not point to superior outcomes in relationship to the cost of care [Institutes of Medicine, 2010 (IOM)]. To address this ongoing concern the Patient Protection Affordable Health Care Act [PPACA (2010)] provides structure for fundamental reform in health care delivery. This new paradigm in health care delivery includes three integrated models of care that are patient centered spanning across the continuum [American Nursing Association, 2010 (ANA)]. The profession is transforming a practice that historically centered on the delivery of acute facility based care to one that is health and wellness oriented. Health care systems currently view patients through a narrow lens, only accepting accountability for care of the patient regarding the immediate episode (Porter, M. 2010). The PPACA shifts the focus for care along the continuum. Concentrating on both the individual patient and the community, nursing is set to measure and achieve quality outcomes through diversity in roles and settings, but will need to continue to gain support of policymakers to remove the barriers to achieving these goals (IOM, 2010). Through new laws such as the recently presented Home Health Care Planning and Improvement Act of 2015, Advance Practice Registered Nurses will be granted a full scope of practice across the nation (American Association of Colleges of Nursing, 2015).
