There are many ways to exchange patient information, direct exchange, query exchange, consumer-mediated exchange, and health Information Exchange. HEALTH INFORMATION EXCHANGE is a system that can be used to share patient information electronically across different hospitals and organizations. The people that use this system include, but are not limited to, doctors, nurses, pharmacists, and other health care providers and patients. The health department where I live uses this. Anytime that I have been in the hospital or went and got a shot somewhere other than the health department they have all the information in my health record; information from the hospital and information from the doctor’s office here in town.…
A very helpful resource my preceptor utilizes is the ASCCP.org website. ASCCP stands for American Society for Colposcopy and Cervical Pathology. This website provides guidelines and algorithms to help establish an appropriate treatment plan for patients with abnormal pap smears. These recommendations were developed and approved by a steering committee made of national professional organizations, government agencies, and advocacy organizations. They used literature reviews, evidence based practices, and risk-based assessments to develop the guidelines (American Society for Colposcopy and Cervical Pathology [ASCCP], 2013).…
A good resource for the patient is the patient portal available online via the website which allows patient access to their information at different times of the day. Meaningful use has greatly impacted healthcare interoperability by implementing certified electronic health records (EHR). With EHR put into act we are now able to view real time information of patients medical history, past and current medications, immunization dates, any diagnoses or allergies, as well as testing and lab reports. It contains all complete and accurate information to give providers access to evidence based tools when making decisions of a patients care. Some EHRs now allow patients to access web portals to view their own health records and even email their doctors. A major contribution to why meaningful use has helped healthcare interoperability is the fact that electronic health records can be accessible to authorized staff…

Directly observed, participated and managed hospitalized patients in ICU unit. Observed and supervised directly by Rahat Hussain MD, of Memorial Hermann Hospital (1028 bed hospital) in Houston, TX. Obtained direct clinical experience working within an advanced, U.S. hospital under expert supervision. I attend clinics where I gather H&P and presents cases and discuss management plans.…
In week 1, I will discuss “Meaningful Use” and what it means. I will also explain why it is important. I will be answering a question that was provided and why I think it is a good or bad idea to release EHR information to the public. In my conclusion, I will review what I discussed and what I discovered and what is important. First, what does “Meaningful Use” mean?…
I will be self-reflecting my physical, emotional, mental, and spiritual development using the medicine wheel as described by Mussell (2005). Mussell defines the medicine wheel as, “a symbol used to represent the dynamic system of mind, body, emotions and spirit, and the needs related to each of these aspects that must be met for the development of human potential.” (Mussell, 2005, p.115). Self-reflection of the four aspects of the medicine wheel (Mussell, 2005) will help me to gain a better self-understanding and insight.…

From midterm to current my CI allowed me to handle more of her caseload, observe online documentation, and weekly progress reports on her patients. She encouraged questions, and thoroughly explained why certain treatments and activities were better than others. She allowed for open communication of patient interventions, which made me feel comfortable enough to take on more of her case load and contributed to my visual, and hands on style of learning. From my initial visit to current my CI has ensured my environment promotes growth at my clinical site.…
This summer I completed my field experience by shadowing a physician assistant named Adam Strantz. He works in a primary care office in Fremont, Michigan. The office, Spectrum Health Medical Group, was fairly small and included only two PAs and one physician. Before starting my shadowing, I set a few goals for myself to learn during my time there. Each and every one of my goals were met through my time there.…
