Patient Observation

This week I was able to observe how the clinic handles inbox telephone messages received from patients and inbox referrals for new patients when they need to be seen by Becky Godby. Chuck asked me to assist him with a vaginal cyst examination. Dr. Rosengren allowed me to draw up an injection with lidocaine and triamcinolone acetonide (Kenalog), which was performed under his direct supervision. I observed a Papanicolaou (PAP) smear procedure, setup, and collection with theThinPrep Pap test. Under the direction of Raymond, I set up a nebulizer treatment and observed a patient perform a peak flow meter measurement, which must be performed before and after the nebulizer treatment is given. I performed multiple urinalyses and prepared a sample to

Financial aspect: it will not cost anything to develop a unit-specific bedside reporting form and the actualization of performing the bedside reporting. My mentor will be assisting on coordinating with the clinical analyst and information technician department on proper installing the bedside reporting form as part of the daily assessment. We are currently using a software program designed according to the patient’s population being admitted to the hospital. The software program consists of basic nursing focused assessment. Therefore, the daily assessment tool bar will just need to be enhanced where there is an assigned tool bar system for bedside reporting to take placed.…

Throughout the entire semester, I incorporated evidence based practice into my clinical days. Whether if I was performing a skill or if I was observing my nurse do a skill, I was thinking is this evidence based. I observed nurses use water instead of alcohol to clean an area on the skin before pricking a patient’s finger for a blood glucose check; this I knew was evidenced based. Some none evidence based thing that I observed were using 5/8inch needles when the nurse should have been using a 1inch length or not using two hands when assessing a patient’s fundus. I met my midterm goal of telling the nurse nicely that this is not what we learned in school.…

When you visit the doctor’s office a person doesn’t think about all the steps it takes to process your visit and what the office does to receive payment from the insurance company. As a patient, the focus is seeing the doctor, telling him or her the symptoms, getting the doctor’s assurance that your health will be back to normal in a few days to a week and getting a prescription for medication. There is a process to making the following steps work smoothly and precisely. The procedures begin with preregistering the patient, establishing financial responsibility, check the patients in, reviewing coding compliance, reviewing billing compliance, check out patients, monitor payer adjudication, generate patient statements and finally follow up payments and collections. Each step has to be done thoroughly by attaining the information necessary to process the claim for the care provider.…

Being at the Phoenix Center today made me see how people’s attitude can affect a person. Patient’s in detox centers such as Phoenix have set schedules to abide by as part of their treatment plan. It is nesseccary for them to attend to these private and group meeting to help them stay on track. Many of them see these meeting as pointless and stupid but they are very beneficial to their progress. These patients are also grogy and tired as they are coming off of drugs and alcohol, and the medications they take also make them tired as well.…

It is not uncommon that patients are discharged early and return to the emergency room with symptoms of leakage after several days. The patient delay that is created has far-reaching consequences, with late leaks (>5 days) being associated with more complex treatment and adverse outcome. It is of upmost importance to treat this complication as early as possible, and thus establish an early diagnosis. Clinical observations could be of more importance in speeding up this process.…

    1. When I started in college, I was unsure of what degree I wanted to get. One thing I know for sure is that I wanted to be in the medical field. While observing Occupational Therapy Assistant Fatima deCouto-Garcia, (a BCC program OTA graduate), at The New Bedford Rehab Hospital, we were tasked with focusing on recognizing the performance skill deficits and contextual factors that brought a client to seek Occupational Therapy services. The facility we visited is considered an acute long term rehabilitation hospital and the patients we observed each had complex multi-facetted symptoms, diagnosis and treatment protocols. For the purpose of this reflection we will focus on one patient, a 45 year old woman, Jane Doe, who 6 weeks prior had her left leg amputated above the knee. Jane was described as a homeless, diabetic, suffering from depression. The high level of Jane’s daily methadone dosage was preventing her acceptance to a skilled nursing…

The alternative site that I observed was an in-home program for children with autism. The child I observed was a 4-year-old boy with moderate autism and was mostly nonverbal. The program was privately owned by a woman who had her Ph.D. in Applied Behavior Analysis. The therapist who worked one-on-one with the student was in the process of receiving her Ph.D. in Applied Behavior Analysis. The parents paid $120 per hour for these services, but I think they received some financial aid through the government .…

In our first video of the semester, we watch a three year old boy in a clinic setting. The first activity in the video involved correctly producing /s/ as the goal while playing with toy cars on a toy road. The clinician first demonstrated the activity by putting her car on the track and then pushing the car forward while producing /s/ along the road. The clinician described the /s/ to be the car’s leaky tire, in order to elicit the /s/ sound. Furthermore, when the clinician demonstrated the activity she would emphasize the /s/ sound.…

San Bernardino, the city a grew up in is a very underserved community. The majority of students in the community receive free lunch at school, including myself during high school. By working as a scribe in the emergency rooms in San Bernardino, I have been able to observe just how underserved this community is. The majority of patients we see have manageable diseases that can easily be controlled with medication, however, they come in to the ER with complications from not being able to receive their medications. For instance, one patient was brought into the ER after a syncope episode without head trauma.…

Happiness and independence are the results of the basic concept of something as so simple as movement. Movement is a task we take for granted; it is a piece of our life that makes us who we are. As an individual, we strive for independence and being able to move our body gives that to us. After finishing my first year as a physical therapy student, I have had the privilege to observe in two different clinical settings. I was able to learn from all different types of age groups and how each responded to therapy.…
