Treatment Plan Development with Goals and Measurable Objective The goal was to treat the residents Peripheral Vascular Disease in her toes immediately so there would not be any amputations. After the resident’s treatment and rehab, the goal was to send the resident home with her daughter. The objective was to find a service and solutions for the resident and family to find a treatment center. Social Services researched and called local treatment center that treat Peripheral Vascular Diseases. After finding the service that would help the resident, social services planned a care plan meeting with the family and interdisciplinary team.
