Patient Intake Assessment

Improved Essays
Corporation for intake and assessment for medical case management. The client is set up with medical care through CMC Myers Park's ID Clinic and receives an appointment prior to leaving the appointment. The client is also assessed for other psychosocial needs at the time of the appointments. This includes housing, treatment for substance abuse, alcoholism and mental illness. At the conclusion of the intake session, the client is given five appointments over the next 90 days in compliance with the Antiretroviral Treatment and Access to Services (ARTAS) Model, that will be implemented along with existing linkages of care provided by the Bradley Reid Corporation.
