Evaluating the genogram of the patient, I notice some diseases, illnesses, and maybe mental problems. In the patient’s family, the diseases that have been seen are: Parkinson’s disease, Hypertension, Stroke, Dementia/Alzheimer’s, Urinary Incontinence, Diabetes, High Cholesterol, and a possible suicide. The diseases from the list that appeared more than once in different family members were hypertension, stroke, and diabetes. Those are the diseases that I want to focus on and give the patient more information about and teach about how to prevent these diseases. I will also give information on all other diseases seen, but go into more details for the more common diseases.
