Patient seen today for PT initial evaluation. Subjective part of examination went well with no complaints. Patient reported pain at rest 0/10 and on activity(walking) 7-9/10. During Physical examination, on active hip and knee flexion, pt complaint of severe pain and locking of left hip joint, stating left hip is out of place. On assisted hip extension to neutral, pt stated left hip relocated back in place and pain decreased. Patient's pain level returned to baseline after resting. Patient's daughter was informed about the incident. Pt advised to go to hospital or consult her doctor, pt declined stating she is felling better after sitting. Pt was advised to consult doctor or go to hospital if needed. Pt's daughter agreed to consult doctor
