Patient Discharge Plan Analysis

A patient discharge plan that is individualized is essential. In this discussion, I will review two learning objectives that I would include in the discharge plan for “Tina”, our Shadow Health digital patient. One learning objective I would focus on is signs and symptoms of hypo- and hyperglycemia and what to do if one of these situations occurs. Tina is going home on insulin and did not have this prior to admission. Managing her diabetes is going to need to be a priority. I would use one-on-one discussion with a supportive and encouraging approach. I would have printed materials/handouts and allot enough time for instruction, questions and concerns, and repeating of information back to me. It is a planned activity specifically designed for her learning needs and has been shown to positively affect patient compliance (Bastable, 2014). I would evaluate if she is grasping and learning this objective by seeing if she asks appropriate questions, is able to accurately tell me the common signs of hyper- and hypoglycemia, and is able to verbalize correctly what she would do if one or the other happened. Hearing the patient’s questions will let me know how she is processing the information and if there are deficits to be addressed. Having her identify and verbalize signs and symptoms of hyper- and hypoglycemia and what to do in each situation will help her retain the information and also feel assured that she can handle it confidently and know what to do. A potential barrier in her meeting this objective would be if she is under stress from recent hospitalization, having dressing changes, dealing with mobility limitations, adjusting to new medications, and other items related to diabetes and hospitalization.
