I am glad to say that she has not had any re-occurrence of her condition in the past decade, thanks in part to the coordinated approach initiated by her PCP. The overarching goal of physicians in healthcare is improving value for patients, where value is defined as the health outcomes achieved that matter to patients relative to the cost of achieving those outcomes (Belliveau, 2016). Without the improvement of value, it will be difficult to accomplish integrating patient care and all efforts will be in vain.
