DOI: 9/1/2014. The patient is a 38 -year-old female licensed vocational nurse who sustained a work-related injury to her low back while working with patient. The patient is subsequently diagnosed with sprain of ligaments of lumbar spine, initial encounter; low back pain; and strain of muscle, fascia and tendon of lower back, initial encounter. As per office notes dated 7/14/16, the patient states that she has completed 5 sessions of physical therapy. It has helped her reduce her symptoms. She has an appointment on 8/4/16 for epidural injections. She sees improvement with acupuncture.
