Patient: A Case Study On A Parking Basis

DOI: 2/4/2016. Patient is a 58-year-old male technician who sustained injury when he tripped over cement on a parking post in the garage and fell. Per OMNI, he was initially diagnosed with injury to both shoulders and knees.
Based on the latest medical report dated 04/06/16, the patient presents for reevaluation of his left shoulder pain. He still has stiffness and pain, especially at night.
On examination of the left shoulder, he remains tender around the shoulder but less significant and he has better active range of motion. He continues to have pain with resisted motion. There is some pain with cross body reach.
As of this report, X-ray of the left shoulder on 2/8/16 (unofficial) shows no acute or chronic bony changes.
