This is a 65-year-old, Hispanic male who has a history of occasional reflux symptoms who underwent endoscopy in 11/2016. During that endoscopy he had white esophageal plaques and he is here to review pathology. Clinically, he has occasional acid regurgitation depending on type of food, but for the most part, he is not having significant reflux or dysphagia. In addition, he voices concern over urinary frequency especially nocturia, but no dysuria.
PHYSICAL EXAMINATION
Normal.
ASSESSMENT AND PLAN
Chronic reflux. Pathology is consistent with hyperkeratosis. Recommendations are to continue his current dose of omeprazole to suppress symptoms. A follow
