Discussion: Lynn (therapist) reported that Dashawn is doing well at the program. She stated that Dashawn has been polite, interact appropriately with his peers. She informed the team that the program is teaching Dashawn how to utilize his coping skill effectively. She informed the team that Dashawn will be discharged from the program on Wednesday 11/23/2016. Lynn (therapist) made the following recommendations: therapy, structure, Tutoring and small group classroom.

Emotional wellness: Dashawn will continue to meet with his therapist after transition from foundation Academy

Physical wellness: Mr. Ricky reported that Dashawn eats for comfort and sneaks food. Mr. Ricky reported that he eats healthy but seems to indulge from sneaking food.
