Reason for Referral and Background Information:

Chris is being referred because Federal and State Guidelines mandate that a three-year re-evaluation be completed to determine if the student still qualifies for services, and are the services needed.

Chris was initially placed October 28, 2009 as a second grader. At the end of Chris’s first year of school, at Starside Elementary in Desoto the DeSoto SIT team recommended Chris for an initial evaluation. The primary concern was low academic performance and attention level. It was determined that Chris met the Kansas Eligibility Criteria for Learning Disabilities and that he needed special education services.

Fluency, decoding and comprehension were all low. His teachers reported that his performance on this test was reflective of his performance in the classroom.

Oral and Written Language:

5. In 36 instructional weeks, when given a Writing CMB: Correct Writing Sequence story starter with 1 minute to think and 3 minutes to write, Chris will write 75 Total Words Written.

Chris’s teachers commented that he “works hard” to listen in class; however, Chris often misses the information being conveyed to the students. Chris’s teachers recognize that it is difficult for Chris to respond orally in class. Teacher then began prepping students for their upcoming test. The teacher gave hints as to the topics that the test would address, and suggestions on what to study.
