Drug (Generic): alendronate Trade: Fosamax
Classification (s): Therapuetic: bone resorption inhibitor Pharmacologic: biphosphonate
Dosage:
PO (Adults): Tx of Osteoporosis – 10 mg once daily or 70 mg once weekly.
Prevention of osteoporosis – 5mg once daily or 35 mg once weekly.
Paget’s disease – 40mg once daily for 6 mo. Retreatment may be considered for patients in relapse.
Treatment of corticosteroid induced osteoporosis in men and premenopausal women – 5m once daily
Treatment of corticosteroid induced osteoporosis in postmenopausal women not receiving estrogen – 10mg once daily.

Indications: Treatment and prevention of postmenopausal osteoporosis. Treatment of osteoporosis in men. Tx of Paget’s disease of the bone. Tx of corticosteroid-induced

For low bone mass before and perodiaclally during therapy. Paget’s Disease: Asses for symptoms of Paget’s disease (bone pain, headache, decreased visual and auditory acuity, increased skull size) Lab Test Consideration: Osteoporosis: Asses serum calcium before and periodically during therapy. Hypocalcemia and vitamin D deficiency should be treated before initiating alendronate therapy. May cause mild, transient increase of calcium and phosphate. Paget’s Disease: Monitor alkaline phosphates before and periodically during therapy. Alendronate is indicated for pts. With alkaline phosphatase twice the upper limit of normal. IMPLEMENTATION: PO: Administer firth thing in the morning with 6-8 oz plain water 30 min before other medications, beverages, or food. Oral solution should be followed by at least 2 oz of water. Sallow tablets whole; do not crush, break, or chew. For effervescent tablets dissolve 1 tablet in half a glass (4oz) of plain room temp. water. Wait at least 5 mins after the effervescense stops, stir the solution for approx. 10 secs and drink contents. TEACHING: Instruct pt. on the importance of taking exactly as directed. If a dose is missed, skip dose and resume the next morning; do not double dose or take later in the day. If a weekly dose is missed, take the morning after remembered and resume the following week on the chosen day. Do not take 2 tablets on the same day. Do not D/C w/out consulting a physician. Caution pt to remain upright for 30 mins following dose to facilitate passage to stomach and minimize risk of esophageal irritation. Advise pt to discontinue and notify HCP if pain or difficulty swallowing, retrosternal pain, or new/worsening heartburn occur. Advise pt to eat a balanced diet and the need for calcium and vitamin D. Inform HCP before dental surgery and to use sunscreen and protective clothing to prevent photosensitivity. EVALUATION: Prevention of or decrease the
