We will contact your office at least quarterly to verify the accuracy of the data we have on file, including whether your providers are accepting new patients, street address, phone number, office hours, and any other changes that affect availability to our members. If changes need to be made prior to receiving our quarterly request you may do so by contacting our Provider Network Department at (541) 684-5580, toll-free at (800) 624-6052 ext. 2580 or by email at providernet@pacificsource.com. It is the responsibility of PacificSource to update the Provider Directory with these changes within 30 days. Please note it can take up to 30 days for initial setup in our directory from the effective date of your contract.
