1. Upon discharge, how does TC see his new life as a survivor of a burn accident?
2. What are TC’s short-term and long-term goals for his life?
3. Does the events that lead to the accident make TC alter his future goals or expectations for his life?
4. What were the daily expectation of TC on the farm, prior to the accident? Such as 2-hour or less in farm activities, daily? Or, 4-or-more hours in farm activities, daily?
5. How is TC’s family adjusting to his absences from the household?
6. Is the girlfriend supportive? Is she a part of TC’s future?
7. To facilitate a smooth transition of care, will TC have access to next care providers in his community after rehabilitation?
8. Does TC have transportation to meet with a local mental health therapist to support his community re-entry?
9. What type of support system does TC have to help with coping skills?
10. In addition to family and friends support, is TC willing to join a support group for burn survivors to assist with coping
