Occupational Therapy Case Studies

My case study is about Robert who is a 65 years old male retired police officer and widow 5 years ago. He is right handed with a history of hypertension currently control and congestive heart failure and prior tobacco abuser. Robert lives along in a community gate house property. His has a friend that lives three blocks from Robert’s house and visit during weekend to play poker and have dinner. He also has a daughter that lives three hours from him. His daughter visits him every three months. Robert was having dinner with a friend when he developed suddenly dysarthria and left hemiparesis involving face, arm and leg with onset of difficulty in speaking, with drooling from the right side of his mouth, and weakness in his right hand. His symptoms progress over the next ten minutes until he couldn’t lift his arm and had trouble standing. He was unable to walk and also reported a tooth ache on the right side of his mouth, as well as headache in the occipital region of the head. He has a moderately slurred speech, but not aphasic, his visual field was intact. He had upper extremity paresis. He has decrease light touch sensation on his left upper and lower extremities. Proprioception and coordination were also decreased. Range of motion of all joints appeared to be normal. Muscle grade (MMT) was 2/5 in wrist extension and flexion on the involved side, ulnar grasp 0/5, and radial grasp 0/5. Shoulder flexion and extension is 2+/5, Shoulder adduction and abduction 2+/5, elbow flexion and extension 2-/5. He walks independently with assistance of a walker.
