I enjoyed your paper and felt that I learned a lot! Very useful information for those of us who may have decides how we choose to obtain a doctorate if it becomes a requirement for our career. However, it appears that we may want to wait to make a final decision. According to Chism (2015), “controversies continue to surround the DNP, including the optimal level of scholarship, readiness of the profession for the DNP as an educational requirement for the advanced-practice registered nurse (APRN) designation, and the type of institution qualified to offer the DNP degree” (p. xiii). Identifying other similar works, Rolfe (2011) emphasizes that Christensen’s (2011) typology of ‘knowing-why’ is virtually identical to both his and Brenner’s concept of ‘knowing that’. In like manner, Barbara Carper (1997), develops a similar typology that she denotes as empirical. Carper’s (1997) description of her empirical concept closely aligns to the ‘knowing-that’ components of Rolfe and Brenner’s. Her empirical model consists of retention of facts, descriptors, and exemplars just as one might describe many of the academic learning outcomes of the novice nurse (Carper, 1997). An additional author, Dr. M. Luntley (2011) incorporates 'knowing-that' in the foundations of his nursing knowledge concept of epistemic conservatism.…

The purpose of this DNP project is to develop an evidence-based practice guideline that can be used to improve the nursing practice of sedation management in the pediatric critical care unit. Evidence-based practice is the hall mark of care that nurses provide in the clinical area. The incorporation of evidence-based data into clinical practice has been seen to improve patient outcomes significantly in the clinical area. With the emphasis on evidence-based practice, it is surprising and concerning that the PICU continues to use an approach to sedation management that is not grounded in evidenced based data. Even more concerning, is the fact that an evidenced-based protocol exits for the adult population.…

With the advancement and complexity of the health care system, it is only fair that the workforce and education level of nurses be increased. The institute of medicine insists that the nursing profession has to change in order to catch up with the growing complexities of the healthcare system [George Washington University School of Public Health and Health Services (SPHHS), 2013]. In 2008, The Robert Wood Johnson Foundation (RWJF) and the Institute of Medicine (IOM) established a two-year campaign to change the nursing profession (IOM, 2010). Also, nurses are needed in most healthcare settings be it a hospital, nursing home, community clinic, doctor’s office, and schools. This wide range of the nursing profession makes it unique and important to the society.…

The objective of Doctor of Nursing Practice (DNP) is to “prepare nurse leaders at the highest level of nursing practice to improve patient outcomes and translate research into practice”, while Doctor of Philosophy (PhD) “prepare nurses at the highest level of nursing science to conduct research to advance the science of nursing” (AACN, 2014). The doctorate degree in nursing would expand my ability to be an effective educator because DNP is considered a practice doctorate and is inclusive of all advanced nursing practice roles: nurse practitioner, nurse midwife, nurse anesthetist, and clinical nurse specialist, as well as nurse administrator, nurse educator, and nurse policy expert. As a future DNP scholar, educator and a leader, I can bridge and narrow the gap between research and practice by using EBP to achieve the clinical effectiveness and benefit of the research (Burson et. al, p.60-73). Moreover, my role as a DNP leader at Kaiser Permanente would prepare me for the needed knowledge and skills to translate EBP into practice by improving the process that would benefit the much needed change in our health care system. These changes would not only promote cost savings but also improving health care.…

However, the scope of practice acts as a barrier to nurses with an advanced degrees, particularly advanced practicing registered nurses (APRNs). APRNs are RNs who have obtained a master’s or doctorate degree in nursing, and have passed a national certification exam at which they are qualified to work as a nurse practitioner, clinical nurse specialist, nurse anesthetist, or a nurse midwife at which they deliver primary care and preventive care to the public (ANA, n.d.). APRNs are also certified to “assess, diagnose, manage patient problems, order tests, and prescribe medications” (NCSBN, n.d.), so they deserve to have the right to exercise their knowledge to the fullest of their capacity. This limitation imposed by the scope of practice is what the IOM report mentions, because health care professionals, like APRNs, are obligated to work within state limits and not based on acquired education and training. With the cooperation of Congress and the legislature, the nursing practice can evolve to adequately deliver optimal care to patients, especially since more individuals are eligible to receive health insurance from the Affordable Care Act.…
