Observation Of Reinstatement

The patient was admitted on 12/5/2013. He was last seen on 11/2/2016 to be reinstated into treatment. Since then the patient did not return and complete the reinstatement process. The patient provided with counseling sessions, random urine screen testing, group sessions if needed, and any other case management the patient needed. The patient came to the clinic in need of treatment for her long term opioid dependence. Please note, the patient was non-compliant with the clinical intervention of attending an IOP group on a consistent basis. The did not meet his objective goals to avoid his usage of illicit drugs. Furthermore, the patient is discharge due to his numerous AWOL status which led to his suspension than discharge. Before his discharge,
