Pain is now considered the fifth vital sign. As nurses, it is our responsibility to continually assess for pain on a regular basis, provide necessary interventions to treat or reduce pain and continually reassess the effectiveness of our nursing interventions after implementation has occurred to provide the client with comfort and promote quality of life. However, just as important is not only relieving the pain, but also finding potential factors that are causing pain and taking action to prevent the pain. It is extremely important to know that pain is completely subjective and know that pain is what the patient says it is.
