Name of Facility: O.Z. lives near to the community resources such as Mid City Clinic, Sharp Grossmont Hospital, City Heights Family Health Center of San Diego, and The Salvation Army Kroc Center.
The rationale for this community resources recommendation and client specific service(s): she lives a few miles away from these facilities. These facilities are all accessible by public transportation. For example, Sharp Grossmont Hospital and Senior Resource Center provide a free service under a project called Community Action to Reach the Elderly (CARE). Seniors and aging classes, and support groups, free service for seniors or disabled adults who live alone. O.Z. is a 69-year-old female, with a history of chronic back pain. She has been taking medication Hydrocodone-Acetaminophen 5-325mg tablet as needed, and her pain is well controlled. She is compliant about her medication and doctor appointment. The family died of natural causes which O.Z. does not know what natural causes they died from.
Brief synopsis of client’s health status, current and current health concern: O.Z. has no current health concern at this time. She has been seeing a current doctor at Mid-City Clinic for six years. Her health has been stable and feels well.
Educational need for current health concern: O.Z. is very aware of own health. There is no current health concern that indicates needs of education. She is following her doctor recommendation and eating healthy and exercise regularly.
Potential health concern: O.Z. is a fall risk because of her right leg shorter than her left leg. Also, her medication can cause constipation, drowsiness, and fall. Due to her age, she is at risk for breast cancer and she denied performing the monthly self-breast
