In the beginning of my journal, I wrote few strokes about my journey as a nurse since it can reflect my understanding of the nursing profession, learning strategies and expectations.
Beginning of journey
Nursing is my second career and I did not work in the medical field while in nursing school. Finding the job with an associate degree without previous experience was not so easy. Applying to multiple hospitals brought no response, so rehabilitation facilities were my second choice. I received phone call next day I filled up the first online application and started working as an RN in a rehabilitation center in few days. Even though facility, where I work, includes subacute units and long term I can tell that my adjustment to a new role as

My assessment findings on admission revealed that patient was AOx3, BP was around 130/70, HR below 90 bpmin, BS was below 200, no complaints of pain or discomfort, no SOB, lung sounds were clear. Next day I received a report from morning shift nurse, the nurse told that new patient is stable. Then I checked my patients at the beginning of my shift. I came to the room of patient that were admitted on the previous day. The patient was in his bed, smiling, he was eating his snack, he told that he spent his night sleeping good and he does not have any pain or discomfort. The patient asked me to close the door when I was leaving the room. I explained to the patient that it is better to leave the door opened to provide his safety and give nurse and CNAs ability to monitor the patient. We agreed that door would be remained open. I left the patient in his room watching TV. In about 15 minutes CNA approached me asking to help with patient repositioning in bed. Nursing supervisor was doing rounds on my floor at this moment so he went to help me to positioned the patient. We found patient slid to 45 degrees from the center of the bed, he complained of difficulty breathing. While we were positioned patients higher in his bed he stopped talking and lost consciousness, his skin turned pale, we immediately started CPR. Unfortunately, we were not able to revive the

It seems as though the patient was discharged from the hospital instead of being admitted and monitored. With the patient’s presenting symptoms, sustained rapid heart rate and abnormal lab values, more care should have been offered to her at the hospital. With respect to Dr. Dorrough it seems in hindsight the patient was misdiagnosed by him and because of that they did not do critical treatments such as starting heparin or another blood thinner or performing an echocardiogram. The initial evaluation and treatment was what needed to be done differently to achieve a different and better outcome from this case.…

When the incision was about to happen at that time the nurse reminded that not to give the antibiotic because of the problems she faced after the surgery in her earlier case. If the nurse would not have been there in the earlier conversation then the doctor was about to give her a drip. In the last case, the patient was suffering from thyroid and it had to be removed because of potential cancer. She met the anesthesiologist before surgery and told him about the breathing problems she faced during her previous couple of operations and she had required additional oxygen for several weeks and in another instance she was shifted to intensive care because of lack of oxygen. However, the surgeon who was about to perform the surgery was unaware of the patients problem before they went through the checklist.…

This case didn’t shock me when I read because I had the similar scenario witnessed in my clinic two years ago. I started my job at the clinic and was assigned to shadow “the best” LPN with 30 years of nursing experience. Before I start, I need to admit that I learned a lot from her. At first, I was impressed by her knowledge and confidence in her nursing job. Her interaction with patients, knowing them and their parents/relatives, and her easiness performing her duties were amazing example for me.…

This scenario also indicates the LPN acknowledges the patients alarm regarding low blood pressure and oxygen saturation level, no indication of intervention is done just the alarm is reset and blood pressure rechecked, the scope of practice of the LPN does not include assessment of patient. There is no documentation in this scenario that the LPN advises the RN or a physician an evaluation of Mr. B. is indicated due to low monitor readings of vital signs. The LPN should have notified either the nurse or physician about Mr. B.’s change in vital signs, so that he could have been evaluated and intervention performed to correct the problems right away. The patient load grows rapidly and the staffing level does not change. The scenario indicates multiple patients are awaiting discharge, these patients can become inpatient while the nurse needs to focus on the higher needs patients in the ER…

The nurse personally reviewed the patient but no proper documentation of her observations even though there was no improvement in vital signs [Conyard, 2015]. This clearly breach standard 1.6 of NMBA standard of practice as documentation defined by Barnett, Treas, and Wilkinson (2016) as a legal evidence where legal experts will examine if a dispute about client care arises such as interventions were timely and appropriate and a tool for communication between the multidisciplinary team about the patient’s status and…

Earlier during my assessment of Mrs. Reyes I learned that she does not have good eyesight, and does not wear her eyeglasses anymore. Therefore, told her that I will talk with the social worker about setting up an appointment with the Ophthalmologist who is covered by her insurance, in close proximity to her house, to ensure that she regularly gets her eyes checked, gets a new pair of eye glasses, and is educated on the importance of eye exams to prevent retinopathy. When the PCA told me Mrs. Reyes’s vital signs, the first thing I noticed was that her oxygen level was 88% on room air that is a bit low. Therefore, I went back in, reassessed our patient and noticed she needed a boost in bed. After boosting I checked her pulse ox, and it had gone back within normal limits above…

Personal Philosophy In December of 1992, after graduating with a diploma from Long Beach City College in California, I began my career as a Licensed Practical Nurse (LPN). Compassion and caring became the cornerstone of my nursing philosophy during my LPN educational track. The Registered Nurse population is encouraged to learn the books and problem solve, while the LPN population was taught to learn the books, but to have an excellent, caring and compassionate bedside manner. In 2007 I received my Associate’s Degree from Cincinnati State Technical School.…

Collaborating with the client is very important in the nurse – patient relationship as we need to work hand in hand to achieve the planned goals. I had this patient who had a Congestive Heart Failure, who was experiencing difficulty of breathing, with productive cough and was restless. As a nurse, following the nursing process, collaborating with the client at this point was addressing his complaints, attending his needs and concerns and for me to do that, I did an assessment for both subjective and objective data. Also, I gave my priority nursing intervention which was to position the client on bed comfortably in a fowler’s position and ensured that he was receiving an adequate amount of prescribed humidified oxygen. Afterwards, I informed my Clinical Instructor about it and of course the staff nurse on duty and verified my assessment.…
