Our priority today was patient education on the expected pancytopenia a day after his autologous bone marrow transplant.
Did it remain the same once you assessed your patient and family, or did it change? If it changed, how/why?
It did remain the same once we assessed the patient, however, the patient self reported emesis occurred after his breakfast, due to a rapid intake of his food. Patient also reported not being nauseated and felt better afterword. Patient also refused nausea medication when proposed.

Safe Practice
On Tuesday I did observe two nurses change a dressing on their patient port. At the beginning, I was surprised by the fact that they needed two nurses to change a patient dressing. I learned that it was not only by UW policy and also for the patient and the nurse’s safety. I was also impressed by the excellent communication, teamwork ship between these two nurses. I felt like I can be successful in this kind of work environment. The nurses were meticulously washing their hand in and out of the patient room.
Also, after report, Nurse goes to the patient room to check the patient lines and make sure that the patient is safe .
Unsafe Practice
On Thursday, I had the tendency to go into our charting area without washing my hand; in my head I did wash my hand already when coming out of my patient room. But, I had to remind myself twice to wash my hand by rule no matter how many times I did that before.
