Patient is being seen today for H & P as prerequisite for admission into residential chemical dependency facility for Heroine use. His mode of use is IV and last use was the beginning of May 2017. Patient reports he is a social drinker and has not had alcohol for months. He smokes 2-5 cigarettes a day. He has been a smoker for 8 years on and off. He declines smoking cessation program. He has a hx of Depression, ADHD and Anxiety and chronic left ankle pain from an old injury. He has been taking Gabapentin for his pain and it has been helpful, however he has been of for a few days since he came to treatment. He reports that he is not currently taking anything for his mental health and he has not seen any psychiatrist for a long time. He reports
