Angela is a 60-year-old single woman who has a condition named Glaucoma. According to the National Eye Institute Glaucoma is defined “as a group of eye disorders that lead to progressive damage to the optic nerve. People with glaucoma can lose nerve tissue, resulting in vision loss”. Angela encountered this condition a few years ago and she has been receiving constant treatments from her optometrist. Her optometrist recommended some medications to help relieve the pain and prescribed sun glasses specified for patients with eye conditions. Recently, Angela encountered a problem while walking up the stairs in her house. Angela fell down a couple stairs and her kids immediately rushed her to the hospital. The children are on vacation to visit their mother and were thankful they were around when she suffered her stroke. Angela has never had signs of stroke previously in the past so it was a shock to her kids to hear that she suffered one. Angela is unemployed as she was fired from her recent job due to lack of competence she says. Angela is not medically insured so she struggles to pay for her hospital's expenses. The case management goals for Angela will be very detailed and specific. First, the case manager will set up a meeting with an interdisciplinary team consisting of an optometrist, nurses, occupational and physical therapists and a nutritionist. Each health professional has their specific responsibility to help restore Angela to good health. An optometrist goal for Mary will be to set up daily appointments monthly for her checkup on her eyes. The job of the occupational and physical therapist will be to help Angela regain strength in her body, so she can learn to walk again by herself. Also, a speech therapist to help Mary with her slurred words. A nutritionist will help monitor the food intake Angela absorbs daily, so she is consuming the proper diet. Management approaches in Mary situation will also be needed first finding a stable home for Angela where she can recover fully after she is discharged from the hospital.
