While planning my care plan for my aggregate I choose five nurses I work with in Baker County. Nurses are always a challenge as we do not usually make the best patients. We may know it’s not healthy to smoke, but we can be stubborn in our ways. I will implement the assistance of the local health department to help with the education materials. Baker County Health Department has set up a Community Health Improvement Plan, they I can use to assist with my plan (Health, 2012).
Selected Group
My selected group of five nurses are two men and three women between the ages of 35 to 58 years old. Four have been smoking for over twenty years, and the one who uses Copenhagen stated he started when he was three years old. The group has been raised in Baker County and most of them have lived on and worked on farms most of their lives. I hope to encourage, challenge and educate this group on the dangers of tobacco and the benefits to their lives if they were to quit.

I would like to see all five of the group, wear masks when working on their farms. I hope this group will take their knowledge of wearing the masks home to include their families and friends. I expect the group to take home the information they have been taught and pass it on to others. I want them to especially learn the following.
