It makes total sense. However, I feel scattered when I am in practice. When I am in relations with a client, I forget some of the important steps, conduct my assessments in the wrong order, and forget or not to consider salient data which is necessary in my decision making and nursing interventions. The concepts are clear, however details get scattered when my brain is trying to process all that is happening. I just understand this is learning process will take time to develop these skills. I just feel my brain does not work well in a linear process. I feel I need spend some time coming up with a system for me to remember and strategies for not jumping around. Using the PQRST assessment enables the nurse to figure out where the pain is, what influences it, type of pain, how intense it and if it changes over time.
