The purpose of this paper is to discuss nurse Witte problem with communication among the healthcare team when she is trying to put plans for patients who were on a ventilation at the hospital to go home with home health care. she communicates with them in a firm but encouraging way. " The problem is that nurse Witte is trying to get a plan together with a group of people from many different heath team members from the doctors to dietary on making detailed plans for patients who were on a ventilation in the hospital to go to a home with home health care. These patients have complex medical needs, so they need very specific instructions. The problem is that it needs to happen as soon as possible but not everyone is on the same page. The doctors did not want to follow a manual on how to transition from a hospital to home made by a nurse. Another problem is that that the nutrition concultete is dumping there side of the prjoect on to the nurse since she is heading this upn but she needs them to do it since it is an important aspect of going home. It should be simple but understandable to everyone so they can read and understand it and that there is no confusion. When communicating with other she needs to stand strong. the way she acts is a perception of what kind of leader she is (Ferreira, Maciel, de Castro, & Lemos, 2013). Is she neatly dressed and carries ann authorize voice or is she weak and lets people push her around. For example, dates on what she wants things to be done so that they can be on track. She also needs to be firm in her plan and not let anyone including herself gets sidetracked shows that she is a strong
