Nurse Rockhead Incident Summary

On November 23, 2016, I was informed by nurse Rockhead that a patient in the waiting room threatened to shot the clerk with a gun. The patient was also reported threatened to do harm if caught outside of work. Although I was made aware of the incident until after it had occurred, I followed up with all members involved and security. I also informed my director Mindy Cody of the incident. In regards to active shooter incidence, it is imperative that our staff stay abreast to the real possibility of such an incident. I will be forwarding accounts related to the incident to Ms. Cody.

Upon arrival, we went in the room and met with E.D. Charge Nurse Sharey Selover and the patient, Gene P. Glaser (DOB: 07/02/1963; FIN: #8533214).
