INTRODUCTION
On October 8, 2015, it was brought to our attention that pieces of concrete had fallen from the ceiling in the staff room. We were also informed that the teachers were fearful and would not go inside the room after the incident. Therefore, they were relocated to the Computer Room #1 on the same building.
INVESTIGATION
An immediate investigation by the Building Officer on October 9, 2015 revealed the following:-
A section of the rendering from the roof slab of the Staff Room had fallen and pieces of concrete were on the teacher’s desk and the floor (averaging 10 – 20mm thick).
