A passport valid to travel to the U.S having a validity date no less than few months away from intended time period of stay in the U.S (unless country-specific agreements provide exemptions). If multiple people is included in your passport, every person desiring a visa must submit an application too.

Your DS-160 confirmation page

Valid e-mail address and phone number

If applicable, required documents based by visa class (like a copy of the petition approval for petition-based visas, I-20 for student visas, DS-2019 for exchange visitor visas, etc. more
