On October 31, 2016, at the nurse’s station I observed the night nurse and the day shift nurse provide hand off report. The night nurse informed the day shift nurse that the patient is a 30-year-old female who had abdominal surgery yesterday and is currently on a clear liquid diet; however, the doctors are planning on changing the diet. Furthermore, the night shift nurse informed the day shift nurse that the patient was complaining of a lot of gas related pain and is very anxious and scared when it comes to moving. The night nurse also informed the day shift nurse that the surgical scar is healing well and there are no other concerns. Lastly, the night shift nurse informed the day shift nurse that the doctors would like the patient to walk …show more content…
This communication allows for a better way to personalize the care for each patient as the day shift nurse is aware ahead of time of the concerns the patient may have. The second attribute that was demonstrated during the interaction was organization. The night nurse had her notes organized on a piece of paper and there were no other distractions while the report was being giving allowing for the day shift nurse to obtain all the information she needed. Furthermore, the day shift nurse organized her notes on her piece paper so she knew what she had to accomplish for her patients during her shift.
Overall the hand off report worked really work well. First, the day shift nurse got a quick summary of what had occurred during the night shift allowing for the day shift nurse to be aware of the concerns the patient was having. It also provided the day shift nurse a quick assessment of the patient and her status. Furthermore, it provided important information on what was planned for the patient during the day shift; therefore, allowing the day shift nurse to plan her day out according to her patients’
