Nelda Observation Report

An unannounced monitoring inspection was conducted on this date. I arrived at the operation which is located at 3710 S. Lake Dr, Texarkana about 6:45 am. I was greeted by Versie Crossland, the person in charge. The purpose of the inspection and the subchapters that would be evaluated were explained to Versie. Nelda Shavers, the director arrived at the center approximately 30 minutes later. I then explained my purpose of the inspection and subchapter that would be evaluated.
Preparation for the inspection included reviewing frequency of inspections made, observing the operations compliance history, the operations fee history, background check list and controlling person list, etc
Both director were present. Nelda Shavers qualifies under 746.1501(a)(7) which is non-expiring license and Brittany Shavers qualifies under 746.1501(a)(6) expiring 8/16/17. Both Directors Training was reviewed and found current.
Background check rules and compliance were discussed with Nelda.
Risk Evaluations: none at this time
Risk Evaluation conditions: n/a
Pending FBI: Kayla Matlock, completed fingerprinting on 6/9. Nelda showed me receipt and I gave her information to submit to clear FBI.

At the time of inspection only one van was present due to vans are used between two operation that permit holder has. Discussed with Brittany the director and van driver, how the van drivers account for children enter and exiting at operation and field trips. Brittany showed me a checklist that must be completed and the additional caregiver that is present on field trips must do a visual check additionally to the van driver. The van drivers are Brittany Shavers and D'Andrea Franklin. Back up drivers are Nelda Shavers and Versie Crossland. Reviewed all drivers training and found to be current of

    As for the $11,000 invoice you mentioned, this was for the fire additions that were required by Stockton fire when store 305 converted to 712. This was Angelo Calamia’s project & independent of the fire conversion. Initially, the city of Stockton mandated that because store 712 has an “M” occupancy rating, a full evacuation/ notification fire alarm system would need to be needed in addition to the water flow system installed.
