A near miss is defined as recognition that an event might have led to an adverse event. One near miss that happened was the nurse almost giving the patient an IV with medication that could have harmed him. Another near miss that occurred was when the patient was being discharged and asked the nurse should all the medicine that the physician prescribed be taking together .
