Nathaniel is a 58 year-old single African American homeless man who suffers from a mental illness that lives in the city of Los Angeles. Nathaniel began experiencing delusions, hallucination, and hearing voices when he was a young boy. When Nathanial was approximetly13 years old he had his first hallucination, in which he saw a car that was on fire and that it was driving away. Nathanial’s mental illness has progressed as he has become older.
Nathaniel’s first mental breakdown occured while he attended the school of Julliard in New York. While rehersing he began hearing voices that told him that everyone around him was looking at him and could hear his thoughts. Nathaniel was not able to handle the voices and he ran out of the rehearsal and went to his apartment. At his apartment the voices became worst that he could not handle it. Nathaniel lived with his sister after his mother passed away. He began to hear voices telling him that his sister wanted to kill him. He was so convinced that his sister wanted to kill him that he left Cleveland Ohio and ended up in Los Angeles California. Nathaniel would like medication to control his mental illness, to help reduce or eliminate the voices he hears, hallucinations, and delusions that make him confused. Nathaniel would also like a stable and safe place to live so he may take control of his illness and live a normal life. Family of Origin Nathaniel and his family lived in Cleveland Ohio. Nathaniel is the oldest son and has a younger sister named Jenifer Moore-Ayers. Nathaniel mother was Mrs. Floria Ayers she was a hair stylist who owned her hair salon. Nathanial attend music school in Ohio and learned how to play the cello. Nathanial did not grow up with his father. Birth and Childhood Nathaniel was born and raised in Cleveland Ohio. Growing up Nathaniel attend private classes to learn how to play his first instrument, that was the cello he was a fanatic of Beethoven music. Nathanial was a child prodigy he was very dedicated and passionate about playing the cello. As a child he did not play baseball or football as the rest of the children his age would. Instead he was so dedicated and passionate about music that it was the only thing he did. Nathanel was always well dressed and very respectfull growing up. Marriages and Significant Relationships It is unknown if Nathaniel was married or had a significant relationship. Current Living Arrangements Nathaniel is a homeless man who lives in the streets of downtown Los Angeles near Pershing Square where there is a Beethoven statue that he admires. Nathaniel is surrounded by other homeless people who have mental illness and are drug users. Nathanial owes an old violin that has two strings, which he enjoys playing in the streets of Los Angeles. Nathaniel is a talented musician who is very smart and shy. Nathaniel appearance is unorganized, his dressing style does not match, he wears different hats and masks, and paints his face. During the interview client had difficulty staying on one topic, and would move from one topic to the next without making sense he also mumbles his words when he speaks. Nathaniel is requesting medication, counseling therapy, and housing. Recommended psychiatric evaluation for mental illness with appropriate medication and twice a week counseling with his therapiest for the next 6 months, to adrees his mental state and medication.
Nathaniel will also attend support group twice a week for the next 3 months to express what he has experienced with his treatment and mental illness. Nathaniel will be provided with shelter informaton and he will apply to obtain a safe place to live in. Client will enroll in career workshop training once a week for the next 3 months to help him look for employment he will learn how to fill out a job application and creat a resume he will also be prepared for a job interview. Nathaniel will get music class for the next 6 months to improve his talent and will play for the church in the community every
