EREPORT # 19225 stated the following: Nancy Ellis reported that the incident happened when Chance was three years old. His biological father broke both of Chances's arms when he was drunk while she was at work. Nancy Ellis did not report any other abuse by the father.
Nancy Ellis reported it to the nurse upon Chances admission to the Acute program. The nurse completed an allegation of abuse form and submitted it to me on 2/20/2016. No, the child has a severe case of autism and has limited verbal skills. Nancy Ellis reported to me that in 2005 they went to court about the incident and at which time Dewayne Hoge signed over all of his parental rights. Nancy reported to me that Dewayne's name was even taken off of the Chance's birth certificate. She reports that all of this is behind her and her children, and Dewayne Hoge can't come within 1200 feet of her or the children. She states that she does not want to rehash any incidents which would drag him back into their lives, and dragging

Chance has autism. Since his mother has a new grandchild chance behaviors have increased. Nancy has reported that she does not know what to do. Chance has been brought into Diamond Grove Center for treatment by ambulance.Nancy states that Chance has never been in a school because of his behaviors and the schools want help because they are scared of what he might do when he is around the other kids. The child has limited verbal skills,and is totally dependent on his mother.
Chances level of functioning is very low. The child is in his mother and stepfathers care,and the biological father is no where in the picture..
Nancy communicated to me she does not want to open this case back up, and that the courts have taken (his) Dewayne Hoge parental rights and she is alright with the verdict.I told Nancy Ellis that I would indicate this in my report that she has told me this information, but I had no control on how DHS would aproach this
