During the NIHSS assessment, she was looking around in the room and resting quietly in her bed. When asking patient the month and their age, she was not able to answer the questions due to aphasia and her cognitive disability, but she was able to open and close her eyes with simple commands. Tammy was able to follow the moving fingers and penlight with her eyes, but she was not able to tell you if she sees fingers in all visual field due to aphasia. Tammy also had a right facial minor paralysis from a previous stroke (3 years ago) and limited left arm movements due to contractures from a previous stroke.
