• Western Hills: o The nurse spent about half of my observation at the computer. This was spent reviewing charts, the schedule, documenting, and scheduling patients. o The nurse spent the first hour before patients arrived preparing face sheets for the provider to use when he went into the patients room. o After the nurse greeted and assessed the patient, she would spend about 10 minutes charting the information from the assessment, and any changes in the patient’s health history or medications. The nurse in this setting demonstrates autonomy very often in the clinic setting. The main resource that the nurses mentioned they use to practice autonomy in this setting is to follow the protocols and procedures. They mentioned that there is a protocol for almost everything in the clinic. Therefore, they have the choice on if they want to perform to their maximum ability or not. What I mean by this is that, according to the protocol, the nurse is able to complete a pre nail trimming before the doctor visits the patient. However, one of the nurses does not like to do this because she is nervous that she would do more harm than good. Since it is the nurse’s choice, she chooses not too. The nurse went to have the patient sign informed consent and have the patient explain the procedure back to us. However, during this conversation the patient looked at the nurse with a blank stare. The nurse used her critical thinking skills and asked the patient if she was okay, the patient then told the nurse that she was not supposed to be receiving a nail removal. The physician was only supposed to be trimming her nail. The nurse then went and reviewed the chart, which stated that the nail trimming was occurring, so then she went and talked to the physician. He mentioned to the nurse that he forgot that they had a change of plans in the patients care. The patient ended up not having to get her toenail removed and got to go home with just a nail
