QP provided MY-Kayla with a CBT activity geared towards mood management. QP explained to My-Kayla that the activity will help her to understand negative emotions, help her to identify automatic thoughts that leads to negative emotions and develop ways to counter automatic thoughts and relate negative moods with cognitive and behavioral changes. QP asked My-Kayla to list some negative feelings and thoughts that leads to negative emotions. QP discussed with My-Kayla ways to counteract negative emotions. QP discussed with My-Kayla, how she can better manage mood. QP examined with My-Kayla automatic thoughts that exacerbate negative mood. QP assists, if goals on PCP continues to be effective. QP reviewed each goal with My-Kayla and her dad to ensure that they continues to be measurable and obtainable. QP gained responses that related to updating interview questions. QP asked questions to ensure that all changes to interview questions were documented and dated within the PCP. QP review with parent crisis plan and discussed any changes that should be made including contact information. QP gained necessary signatures. QP gathered information from My-Kayla and her dad to update NC Topps. QP discussed My-Kayla grade and behavior in school to complete the academic section on the NC Topps. QP assess if consumer engaged in any substance or alcoholic use. QP gathered information from parent about history of abuse in order to effectively answer NC Topps interview questions. QP discussed services and assesses whet services is needed. QP ended the session by reminding My-Kayla and her dad of the next scheduled
